You’ll probably notice that my questionnaire is a little more detailed than most—and I promise there’s a reason for that! Home birth care is incredibly personal, and I want to know a little about you, your pregnancy, your health and birth history, and what you’re hoping for before we ever sit down together. Every question on the questionnaire is there for a specific reason; none of them are arbitrary or included just to make you fill out more paperwork. Each one helps me better understand you, your needs, and how I may be able to best serve and support you during your pregnancy and birth. It also gives me a heads-up about anything we may want to talk through more carefully during our consult and helps us both get a better sense of whether we’re a good fit for one another. Having that information ahead of time means we can spend more of our consult actually getting to know each other instead of working through a long list of basic questions. Choosing a midwife is a big decision, and I believe the fit matters on both sides. Taking a little extra time on the questionnaire helps make our conversation much more meaningful.

Please allow up to three business days for a response to your inquiry. As a small, relationship-based practice, I personally read through and respond to each message while continuing to care for the families already entrusted to me.
