Charis Birth & Wellness LLC (“Charis Birth”) takes your privacy seriously. This policy outlines the type of information we collect and how it is used.

INFORMATION COLLECTED

Charis Birth collects contact information such as your name, phone number, address, and email. If you are inquiring about midwifery services, we will also ask for the estimated due date of your pregnancy and general information about your health. If you are inquiring about lactation services, we will also ask for the age, sex, and birthday of your infant.

COLLECTION METHODS

Charis Birth may collect your information from contact & inquiry forms on our website, scheduling app (Acuity), phone calls, emails, service contracts, and text messages.

DATA USE

All data collected is used for the purpose of responding to inquiries, providing services directly to our clients, scheduling & confirming appointments. Email addresses may be used to send follow-up instructions & information related to the care you are receiving from Binah Birth, such as information about testing that will be offered to you at your next prenatal visit, infant feeding plans, and/or lab results.

Email addresses may be used for communication of upcoming events of interest such as monthly support group opportunities for postpartum clients.

Telephone numbers may be used to send appointment reminders via text message.

DATA STORAGE

Different parts of your data are stored in various locations. Data obtained via our website is stored within our Google Drive, which is HIPAA compliant. Your name and phone number may be stored locally in Grasshopper (our telephone app). Emails & phone numbers are also stored within Calendly (our scheduling app) when an appointment is scheduled by the client. Clients who opt-in to receive information about upcoming events will also have their name and email address stored in Mail-Chimp. If a client opts-out, their data is removed from the Mail-Chimp system.

DATA SHARING

Client information is never shared with third parties/affiliates for marketing/promotional purposes. Your data may be shared with Acuity for appointment scheduling & Mail Chimp for communication about free support groups that we host.

All sharing mentioned in this policy excludes mobile opt-in and consent; opt-in information is never shared with anyone for any purpose.

CHANGING, CORRECTING, OR REMOVING YOUR DATA

To make changes or corrections to your information, please contact us by texting XXX-XXX-XXXX or emailing midwife@charisbirth.com.

To request your data be removed from our system, please send an email to midwife@charisbirth.com.

Messaging and Data Rates may apply. Your mobile carrier may charge fees for sending or receiving text messages, espeically if you do not have an unlimited texting or data plan.

You may Opt-Out of text messages at any time by texting, emailing or replying STOP or UNSUBSCRIBE to midwife@charisbirth.com or XXX-XXX-XXXX. After unsubscribing, you will receive a final SMS to confirm you have unsubscribed and we will remove your number from our list within 24 hours.
